About the Sharecare Program

To support providers to deliver effective care coordination, BCBSAZ offers an integrated program of health and wellness tools in conjunction with our partner, Sharecare. This program consists of resources and services geared toward educating, engaging and empowering members to make healthier lifestyle choices.

See below for specific information about the various program components.


Please note: Not all wellness services are available to all members. Available services will depend on the member’s benefit plan type and personal circumstances. Some services are provided through independent contractors.

Sharecare Health & Wellness*

We have partnered with Sharecare® to bring employers a truly differentiated digital health and wellness experience. Our members can expect immediacy, simplicity, and relevancy in a mobile app, while employers will find tools that drive sustained employee engagement to improve health outcomes and control rising costs at azblue.sharecare.com.

RealAge Test Health Assessment

This online assessment tool is available to all members (except Medicare & Medicaid) and consists of a series of questions covering eating, exercising, and sleeping habits, along with family health history, behaviors, and existing conditions. The assessment report is generated automatically online and measures the member’s physiological age (their “Real Age”) vs. their chronological age, along with specific indicators of healthcare risks and simple steps to reduce risk.

The assessment requests some health measurements from the past six months. This includes current height, weight, blood pressure, and from recent lab work: total cholesterol, HDL, and fasting blood glucose.

Walker Tracker®

The program allows participants to track their steps and activities and set personal wellness goals online. Program participation is open to members of BCBSAZ employer group benefit plans. The website includes tips and resources to help participants stay motivated so they can reach their personal fitness goals.

Nurse On Call*

Members can connect with a nurse 24/7 to get answers to questions about symptoms they are experiencing, minor illnesses and injuries, medical tests, or preventive care, as well as suggestions for next steps based on their situation.

*Nurse On Call is available to eligible members. Members under age 18 must obtain parental consent prior to using the services. Services and treatment options presented may not be covered under the member’s BCBSAZ benefit plan. Members should review benefits or contact BCBSAZ at (602) 864-4400 or (800) 232-2345 prior to receiving non-emergent services. Certain services are provided by an independent third party contracted with BCBSAZ to provide health enhancement services to BCBSAZ members.

Lifestyle Coaching*

The program offers an assigned health coach who will provide a member with support in achieving their health goals through lifestyle and behavior changes. The program includes one-on-one health support and guidance with setting goals and learning to maintain healthy habits.

*Available to groups with 100+ employees or more that have Lifestyle Coaching through Sharecare. Lifestyle Coaching is not available for Medicare supplement members or FEP members. Availability of services and programs may vary. Certain coaching programs, have eligibility requirements. Programs are provided by an independent company who has contracted with BCBSAZ to provide specific services to Blue members

Fertility, Pregnancy and Parenting Resources

This maternity support program offers:

Fertility: Helps track reproductive cycles. Includes an ovulation calendar, symptom trackers, coaching and an online community

Parenting: Supports parents with children under four, and helps them track their child’s health and development – features include health assessments, milestone checklists, content and infant activity tracking

Pregnancy: Helps expectant mothers understand, track and maintain both their own and their baby’s health throughout pregnancy. Features include a calendar to support daily activity, doctor appointment and milestone tracking and content and coaching

*The Ovia Fertility, Parenting and Pregnancy Program is available to members of certain large employer groups.

Blue365 - because health is a big deal

Blue365® Discounts*

Members can take advantage of exclusive deals with savings from top health and wellness brands. Discounts are offered on weight-loss programs, health and fitness clubs, healthy travel experiences, and so much more.

Care Management*

The Care Management program offers support to members faced with a catastrophic medical event or diagnosis. Services are voluntary and include one-on-one telephonic communication with a care manager. Services also include assistance with understanding health plan benefits.

Other Services

  • Flu and pneumonia shots may be available to eligible members at community sites through BCBSAZ-contracted providers. Coverage is subject to the member’s benefit plan.
  • Mobile On-Site Mammography (MOM), a BCBSAZ-contracted network provider, offers screening mammograms at various community locations to BCBSAZ-insured female members age 30 and older. Some eligibility criteria may apply. Coverage is subject to the member’s benefit plan.

RealAge Programs Digital Coaching

The RealAge Program encourages members to take small action – establishing tiny habits – to build your confidence and lead you toward achieving your goal. Pick a goal to work on: Stress, sleep, nutrition or activity. Get advice, helpful tips, personalized guidance and more from your phone or tablet.

*Program provided by an independent company contracted with BCBSAZ to provide specific services to Blue members.

Disease Management*

The Condition Management (Disease Management) Program offers support to members diagnosed with certain chronic conditions such as:

  • Asthma
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Congestive Heart Failure
  • Coronary Artery Disease
  • Diabetes







Device Monitoring of biometrics (e.g. blood glucose monitoring) for certain qualified members diagnosed with:

  • COPD
  • Diabetes
  • Heart Failure
  • Coronary Artery Disease

Care Alerts provided added education and build awareness of how best to manage a chronic condition regarding:

  • Prevention
  • Medication Adherence and Safety
  • Gaps in Care

BlueCare AnywhereSM Telehealth

With BlueCare Anywhere, members can connect to board-certified doctors by live video for urgent medical care, psychiatry, and counseling sessions. The BlueCare Anywhere telehealth service is available any day, any time—from a computer, tablet, or mobile device.
